For New Patients

Thank you for choosing the Ayer Neuroscience Institute for your spine care. It is our goal to help make this process as simple and convenient as possible for you and your loved ones.

We understand that a diagnosis involving your spine can be frightening, but we are with you every step of the way to answer any questions and guide you through this process.


Before Your First Visit

Two weeks prior to your scheduled appointment, you will receive a questionnaire on your MyChart account. By completing this questionnaire ahead of time, it will shorten your time spent in the office and help us with your care. If you need help accessing your MyChart, or resetting your password, call the office where you will be seen.

If needed we can mail the questionnaire or you can complete it upon your arrival, but please arrive 15-20 minutes early.


What to Bring

Please bring any recent spinal imaging (such as MRI, X-Ray, or CT Scan) on a CD if possible and wear loose-fitting clothing for your appointment. It is also helpful if you are able to bring a list of any current medications you are taking.
